Home Health Episode Engagement

From Start of Care to Recovery

Step 1

Patient receives a call checking in on their status and asking general questions about recovery status, as well as confirming initial home health visit.

Step 2

The patient receives additional calls that monitor the status of their recovery and provides opportunities to listen to their personalized recordings throughout their episode of care.

Step 3

Should any issue or concern arise on any of the calls, home health care staff members are immediately notified to resolve issues and prevent adverse events.

Step 4

Follow up with patients after their episode to identify those who need re-certification.

Better Patient Outcomes

Checking in on patients in between home health visits ensures patient concerns are addressed before an adverse event or readmission occurs.

Increased Patient Satisfaction

Adding meaningful and timely touch points between home health visits improves patient perceptions and leads to higher Home Health CAHPS.

Enhanced Staff Efficiency

Eliminate the repetitive, manual, and time-consuming task of contacting patients and give staff members back valuable time in their day to focus on care delivery.

With data gleaned from the CipherHealth technology platform, we are able to gain further insight into our patients' health status and behaviors, enabling us to intervene early as problems arise. Ultimately this is leading to reduced instances of hospital readmissions.
