A new whitepaper from the Health Care Transformation Task Force evaluated the key components of care management programs to help improve the quality of care for “high-need, high-cost populations.”
In the whitepaper, the Task Force shares the building blocks for effective care management programs, including patient and family engagement, patient-centered care, transitional care, and care coordination. The paper goes on to describe the areas where care providers have the greatest need for improvement: tailoring care management programs for individual patients, ensuring meaningful patient engagment, and aligning providers to the goals of their programs.
CipherHealth’s customizable care management platform, CipherCareManagement, created in collaboration with UCSF, utilizes the best practices identified by the Task Force. CipherCareManagement enables organizations to send and receive actionable information, bridging the gaps between members of the patient’s care team. By aggregating data on a single platform, CipherCareManagement gives care providers visibility into the patient’s longitudinal history, promoting efficient care coordination.
