Product Update: Test Recorded Care Instruction Calls with Patients Before Discharge

Helping patients remember discharge and care plan instructions is key to ensuring patients can safely transition to their next care setting. Empowering patients with personalized recorded care instructions is an effective strategy to provide patients with the information they need post-discharge and can help reduce readmissions, as well as increase patient satisfaction.

With Echo, CipherHealth’s care recording software, nurses and other care team members can record important care information for patients and their families to access from home. With Echo, hospitals around the country are helping to improve patient outcomes and instill confidence in a patient during a critical moment in his or her care journey.

This new feature allows nurses to send a test call to the patient’s number and demonstrate exactly what the call containing their discharge instructions will sound like. The nurse and the patient can listen to the call together and go over any questions the patient might have prior to discharge.

To access this feature, log into Evolve and locate the patient’s page. In the patient’s patient in Evolve, select the microphone icon. When you hover over the icon the title “Record” will appear. After recording the discharge instructions, a phone icon will appear next to the recording. When you hover over this icon the title “Call” will appear. Clicking on this phone icon will allow the staff member to send the example call containing the discharge instructions to the patient. You can see what this looks like on the test patient record in the video below. To see the video, please click on the image.
