The importance of proper post-discharge planning

When a patient is in the hospital there are hundreds of things that happen to ensure he or she is receiving quality care. Between the diagnoses, treatment, and discharge the patient has dozens of interactions with hospital staff, while even more occur between staff members outside of the patient’s room. When a patient is discharged, hospitals are still responsible for making sure they stay on the path to wellness. However, if the hospital does not have a plan for post-discharge care, current workflows leave little to no time for proper patient follow up.

The benefits of following up with patients are plenty. For instance following up with patients after they leave the hospital helps identify patients who might be at risk for an adverse event. This increases patient satisfaction and decreases the likelihood a patient will be readmitted. However, if hospitals are unprepared, the task of reaching out to patients might seem overwhelming. Any follow up that does happen is time consuming,  lacks proper data collection, and will reach relatively few patients.

Proper discharge planning means enhancing, not inhibiting current workflows. Some hospitals use volunteers or call centers to start reaching more patients, while others opt in for an automated solution that triages patients with issues back to the hospital. When it comes to post-discharge planning there are many possible strategies, the crucial aspect is that hospitals are evaluating and implementing ways to perform the most effective outreach to their unique patient population.
