CipherHealth is pleased to announce the winners of our Excellence & Innovation Partner Awards. The Excellence Award is given to an organization who has had extraordinary results in enhancing the patient experience and the Innovation Award highlights organization’s unique programs designed to improve the quality of care.
Congratulations to Baptist Health, Baylor Scott & White at Plano, Middlesex Hospital, and Well Care Home Health!
Baptist Health and Baylor Scott & White at Plano were selected for the Excellence Award. Baptist Health has had excellent outcomes using Voice as exemplified through their stellar readmission rate reduction. Inpatients engaging with Voice had a 12.5% lowered readmission rate, while COPD patients engaging with Voice had a 15.8% lowered readmission rate.
Baylor Scott & White at Plano has exemplified excellence in embracing technology to drive results and improving patient outcomes. For FY 2016, Baylor Scott & White at Plano was able to meet their goal of achieving a readmission rate under 11%. They were able to achieve this success due to their spectacular leadership who are insightful, inquisitive, and strategic in their use of technology, leveraging reports and relationship with their strategic advisor to make targeted program improvements.
Middlesex Hospital and Well Care Home Health received the Innovation Award. Middlesex Hospital’s Center for Chronic Care Management thought outside the box in the implementation of Population Health Voice Programs. Diabetes, Heart Failure, and Respiratory patients are enrolled following an educational class for their related DRG. Moving forward, Middlesex Hospital is planning to partner closely with local primary health providers to begin enrolling a larger group of patients.
Well Care Home Health is a phenomenal innovator of successful home health patient engagement. Since Well Care’s launch of Voice, the organization has seen great patient experience results, has made thoughtful structural changes to continually optimize their program, and has led strategy and best practices in the CipherHealth home health market.
