Announcing the Winners of CipherHealth’s 2019 Partner Awards

At CipherHealth’s Partners In Excellence User Conference in June, four organizations will be awarded for their excellent and innovative use of CipherHealth technologies. As patient engagement continues to drive strategic priorities, these leading organizations are proving to be forward thinking in how they improve patient outcomes and experiences across the care continuum.

The Excellence & Innovation Awards
The Excellence Award is given to an organization that has had extraordinary results in enhancing the patient experience and the Innovation Award highlights an organization’s unique programs designed to improve the quality of care.

This year we are pleased to feature outstanding organizations in both award categories.

Driving Excellence with Collaboration
Our Partners in Excellence award recipients leverage CipherHealth technologies to achieve key patient engagement goals and fulfill their organizational missions. By systematizing the processes and engaging non-traditional users in their organization, the award recipients have achieved outstanding results that demonstrate improved patient outcomes and enhanced patient experiences.

For Cedars-Sinai, excellence was achieved by engaging its pharmacy residents to reach out to patients who had questions about their medications. Using the National Coordinating Council for Medication Error Reporting and Prevention (NCC MERP) harm index, the pharmacy residents conducted a retrospective analysis of the medication issues identified and resolved as a result of the outreach call. This provided needed visibility into the patient’s’ medication issues and risks after hospitalization. Through this program, Cedars-Sinai demonstrated increased HCAHPS scores, reduced readmissions, and reduced harm.

Providence Little Company of Mary, which is part of the Providence St. Joseph Health System, demonstrated excellence in leveraging CipherRounds for purposeful rounding. With its rounding program, Providence Little Company of Mary seeks to find opportunities to enhance a patient’s stay. By utilizing reports to understand the quality of rounding in addition to leveraging resources such as their current volunteer program, the hospital is able to maximize resources and improve the patient experience.

Innovating to Deliver Patient-Centered Care
Innovation is difficult to achieve and requires creativity, trust, and flexibility. The team at Carson Tahoe demonstrated innovation by finding new and productive ways to leverage their CipherRounds technology.

With its all hands on deck approach, the team has developed and implemented rounding scripts in the Emergency Department and Inpatient units with scripts tailored to all aspects of a patient’s care: from “Welcome Rounds” upon admission to EVS and food and nutrition rounds throughout their stay. Carson Tahoe’s Emergency Department, in particular, utilizes CipherRounds alongside CipherOutreach. This powerful integration empowers meaningful interactions and cross-functional service recovery across the entire patient experience.

Mercy Health Select, an Accountable Care Organization (ACO) participating in the Medicare Shared Savings Program (MSSP), leverages CipherOutreach proactively by reaching out to patients with chronic conditions post-discharge to:

  • Reinforce self-management skills
  • Detect an early change in a patient’s condition
  • Alert nursing staff to outreach to the patient and provide early interventions to address that change

High-touch outreach and timely intervention helps to mitigate the exacerbation of chronic conditions and in turn reduce avoidable visits to the Emergency Department or hospital readmissions.
