CipherHealth has launched a new way for patients to engage with CipherOutreach programs! In addition to responding using their phone’s keypad during a voice call or text message interaction, patients can now complete an Outreach survey from their web browser. A link to the survey is delivered to patients or managed care members via text message or email. The benefit of using the web form as an Outreach modality is that the questions are displayed on the web browser screen – making it easier for patients to visualize the question/answer being asked and to scroll back and forth between questions. This is especially useful for longer Outreach scripts.
Ways Your Team Can Use Webform:
- Longer surveys such as those to collect Patient Reported Outcomes (PROs) or Patient Activation Measures (PAM) that may be used in value-based care programs such as bundled payments
- Post-Discharge Follow-Up surveys as a way to send secure surveys via email
How Patients Access Webform Surveys:
Patients will receive an SMS text message or email containing a link to the secure webform survey. When the patient opens the link, they will enter their date of birth to validate their identity before answering the survey questions. Any negative responses or issues generated will be triaged to the designated staff members as per your other outreach programs.
